These Terms & Conditions govern your use of the BurnWell Medical website (the “Site”), operated by BurnWell Medical, LLC. By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

General Education, Not Medical Advice

All content on this Site is provided for general education and informational purposes only. It is not medical advice, diagnosis, or treatment. Nothing on this Site should be used as a substitute for professional medical advice, evaluation, or care from a qualified clinician.

No Provider-Patient Relationship

Using this Site, joining the waitlist, or contacting BurnWell Medical through the Site does not create a provider-patient relationship between you and BurnWell Medical or any of its clinicians. A provider-patient relationship is established only through a formal enrollment process and a personal clinical evaluation.

Individual Treatment Decisions

Individual treatment decisions require an evaluation by a licensed clinician. Any description of services, medications, or programs on this Site is general in nature and does not determine whether a particular treatment is appropriate for you.

Not for Emergencies

This Site is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the waitlist, contact forms, or email for urgent or emergency medical concerns.

The Waitlist

Joining the BurnWell Medical waitlist does not schedule an appointment, guarantee acceptance as a patient, guarantee treatment eligibility, or enroll you in a membership. The waitlist is a prelaunch communication tool used to share updates about BurnWell Medical and future enrollment opportunities.

SMS Terms

We offer two separate, optional text message programs. You may opt in to one or both by checking the corresponding box on our forms. Each program requires its own independent consent, and neither box is selected by default:

• Informational text messages regarding your inquiries, waitlist status, and requested follow-ups.
• Promotional text messages regarding BurnWell Medical services, practice opening, membership opportunities, and special offers.

Message frequency varies. Message and data rates may apply. Reply STOP to opt out of either program at any time, or HELP for assistance. You may also contact hello@burnwellmedical.com for assistance. Consent to receive promotional text messages is not a condition of purchase. Wireless carriers are not liable for delayed or undelivered messages. These text messages are not medical advice, emergency services, or a secure channel for confidential medical information. When you consent, we record which consent category you selected, the date and time of your consent, the page where you provided it, and the disclosure text shown to you at that time.

Memberships, Services, and Availability

Membership details, clinical services, medication eligibility, pricing, and availability described on this Site are subject to final enrollment terms and clinical evaluation. BurnWell Medical reserves the right to update, modify, or discontinue any service, membership, or program at any time.

Ownership of Materials

Website materials, including text, graphics, logos, and design elements, are owned by BurnWell Medical unless otherwise stated. You may not reproduce, distribute, or create derivative works from these materials without our prior written permission.
